Shipping animal samples across customs borders requires additional documentation, depending on the destination country. Below you will find both blank templates and pre-filled form templates. We accept no liability for the accuracy or completeness of the documents provided.
Please note: the pre-filled forms contain only the information that can be derived directly from the sample material itself. Your personal details, information about the animal concerned, and any other individual details must be completed and signed by you.

- The declared value of the individual samples must be stated so that all samples together reach a total value of $1.00.
Health Declaration
Bilingual (DE/EN) veterinary form confirming non-infectious DNA specimens.
Note to the sending party:
- A valid health certificate must always be enclosed, except for samples on FTAcard/GOcard blood cards and hair root samples.
Note to the veterinary staff at the shipping location:
- Please sign the declaration on the first page, as well as the following annex specifying the respective samples, in the designated places.
Note to the border control post:
- Generatio holds a current import permit for the import of examination material (...) for research or diagnostic purposes, which can be requested from Generatio by the relevant border control post if needed. Generatio is happy to send a certified copy upon request.
